3. Requesting Party (Document-owner with Valid ID)
4. If the Requester is a Representative (Authorization Letter of Document owner, his/her Valid ID and Valid ID of representative himself/herself)

B. For Death Certificate

1. Name of Deceased
2. Date of Death of the Deceased

 C. For Marriage Contract

1. Name of Contracting Parties
2. Date of Marriage
