Requirements for Registration of Births (Delayed Registration)
Delayed Registration (Over 30 days from the Date of Birth)
1. Same as required in the Registration of Births within the reglementary period (Items1-3)
2. Any record/document wherein date and place of birth of registrant is indicated
Example:
- Baptismal Certificate
- Voter Registration Record
- SSS Record
- Immunization Card (As in the case of newly born not registered within 30 days)
- Negative Certification issued by PSA (optional)
- Others
3. Informant should either be the parent or the registrant himself/herself provided that he/she is 18 years old or above at the time of registration
4. Current Community Tax Certificate of Informant/Registrant
5. Marriage Contract of the Registrant, if married
6. Joint Affidavit of two disinterested persons who have known about the facts of birth of the registrant and their current Community Tax Certificate
7. If child is Illegitimate*:
- Informant should be the mother and the father is advised to be present during the registration for Admission of Paternity purpose
- Affidavit to Use the Surname of the Father to be executed by the Mother
- Current Community Tax Certificate of Parents
- Valid ID of Parents
- Mother’s Affidavit (forms available from the Office of the Municipal Civil Registrar)
*Illegitimate – a child is said to be illegitimate if he/she was born before the marriage of parents took place or the child was born to parents below marrying age.
