Requirements for Registration of Births (On-Time Registration)

On-Time registration (0-30 Days from the Date of Birth)

1. Accomplished Information sheet (Form available from the Municipal Civil Registry Office)
2. Marriage contract of parents, if available
3. Item for Attendant at Birth in the Certificate of Live Birth must be duly signed by a  physician, nurse, midwife or by a traditional midwife (Hilot) who attended the birth
4. Informant should either be the parent or the attendant at birth
5. If child is Illegitimate*:
   • • Presence of father and mother during registration for Admission of Paternity and Affidavit to Use the Surname of the Father, respectively
     • Current Community Tax Certificate of Father and Mother
     • Valid ID of Parents
