Requirements for Registration of Deaths
- Informant should be the immediate member of the family of the deceased
- Certification from the Punong Barangay stating that the deceased is from the said Barangay
- Information Sheet is available from the respective Midwife of every Barangay
- Certificate of Death duly accomplished (Form available from Civil Registry Office)
- Burial Permit if the deceased is to be buried within this Municipality
- Transfer of Cadaver Permit if the deceased is to be buried outside this Municipality
