Requirements for Application to Use the Surname of the Father
- Authenticated (PSA) copy of Certificate of Live Birth of the child
- Certificate of Live Birth of the Child reproduced in 5 copies
- Affidavit of Admission of Paternity in case the child was previously registered and has not been acknowledged by the father at the time of registration
- Affidavit to Use the Surname of the Father (AUSF) to be executed by the mother at the Civil Registry Office
- Current Community Tax Certificate of father/mother
- Valid ID of Parents
