- All document required to be submitted for the correction of clerical error, shall be submitted also by the petitioner for change of first name.
- Clearance from authorities

- Employer’s Certification/Clearance, if employed (if not, as in the case of a student – School Certifications and if self-employed- Business Permit, not employed – Affidavit of Non-Employment)

- National Bureau of Investigation or NBI Clearance (note the date issued, NBI Clearance is only valid for 1 year)
- Philippine National Police
- other such clearances as may be required by the concerned C/MCR
- Proof of Publication

- Affidavit of Publication from the Publisher
- Copy of the Newspaper Clipping
